What we do:

Our team at CommCare offer safe, confidential, dignified and respectful professional care in the comfort of your own home.   We provide a range of services including government-funded home Care packages as well as services on a private funded basis.

Government-funded services:

If you are eligible for any of our government funded programs, you may be able to access the services you need at a low cost or at no cost to you depending on your income.   Fees are means tested. We can assist you in obtaining an assessment or you can call direct to

1800 200 422 or visit – the national entry point to Australia’s aged care government funded system.

We support your Independence:

Many people prefer to remain in their own home and the community they are familiar with, even if their health reaches a stage where high care is required.   CommCare is able to provide tailored home care solutions to support your

independence and flexible enough to change along with your health care needs.

Privately funded services: 

Get the service you want when you need it, and avoid the long wait period and government paperwork.   Our pay as you go option enables you to purchase services on a one-off or regular ongoing basis.   You may even wish to supplement your government funded support with extra support.

Who we are:

Comm Care are a team of qualified community carers that are dedicated to providing boutique aged care support in your home, whether its personal care, domestic duties, transport, assistance with medication, a helping hand with meals, shopping, companionship or in- home respite –we can tailor your care requirements to meet your needs.  Our personal carers have a minimum Certificate 111 in aged care/community care or equivalent, and we are a government approved provider.

Services we provide:

  • Personal Care:

Showering, assistance with personal hygiene, dressing, grooming.

  • Domestic assistance:

Help with domestic tasks, such as cleaning.

  • Transportation:

Transport to medical appointments or community events

  • Shopping

  • Meal preparation

  • Companionship

  • In home respite care

  • Overnight care

  • Specific support:

Such as dementia support, continence care, monitoring health care needs.

  • Case management:

Case management is the coordination of home care services to ensure your care and requirements are being met.   Our care managers are skilled aged care professionals and will ensure you have access to a range of services that best suit your needs, whether through the funded government home care package or privately funded or even a combination of both.

How much funding does a home care package recipient receive? 

There are four levels of packages available and a person is allocated a package depending on their needs as assessed by the Aged care Assessment Service (ACAS). The below rates are as at July 2018.


*Level 1 - $8,271.00 per year


*Level 2 - $15,045.00 per year 

*Level 3 - $33,076.00 per year 

*Level 4 - $50,286.00 per year
